Appointments and Schedule
Generally, you are seen in our office by appointment. We try to accommodate everyone
for their appointment as quickly as possible. However, we ask that you keep the
scheduled appointment, and if circumstances do not permit you to keep your
appointment, give us at least 24 hours' notice.
- Inform our office of any changes in your insurance and billing information.
- Please, we ask that you arrive at least 30 minutes before your scheduled initial
appointment and at least 15 minutes before your follow-up scheduled
appointment.
Bring the following you're your appointment:
- Insurance card and photo ID
- Referral or physical forms (if any)
- List of prescription and over-the-counter medications that you take regularly.
- Any radiological films and reports related to your visit.
- Co-payment or deductible payment
We accept cash, Visa, MasterCard, American Express, Discover, and all debit cards with any of
those logos.
