DAJA HEALTH provider participates with Medicare and most insurance plans. However, participation varies as changes occur within the insurance industry. Please check with our receptionists to confirm if we participate in your insurance plan.

Deductible Payment Policy

We collect $125 upfront for everyone with a medical insurance policy with deductibles. Please, call your insurance company to verify if your policy includes a deductible. This policy does not apply to annual physicals.
