Palliative Care (PC)
Palliative care focuses on improving the quality of life for persons living with serious or potentially life-limiting illnesses. At DAJA HEALTH, we work with providers, nurses, therapists, counselors, and social workers. The team collaborates with the patient’s care team, the patient, and their family to provide medical, emotional, and social support needed to cope with the burdens of a serious illness.
Care is tailored based on an individual’s medical needs and wishes for how they want to be treated physically, psychologically, and spiritually.
Palliative care is available to anyone with a serious or life-limiting illness. Some conditions appropriate for palliative care include but are not limited to cancer, heart failure, chronic obstructive pulmonary disease (COPD), Parkinson’s disease, Alzheimer’s disease, dementia, end-stage kidney disease, or hepatic (liver) failure.
Palliative care treats symptoms even if the underlying disease cannot be cured. The main objective of palliative care is to relieve the pain, suffering, and discomfort associated with an illness and to reduce patient and family members’ stress. Palliative care is provided in several settings, including the patient’s home, outpatient palliative care clinics, nursing homes, hospitals, or other areas of the patient’s choice and location.
